We specialise in supporting clients with medical complex needs in their own home.
Our Registered Nurses are highly experienced in acute clinical assessment, autonomous decision making, diagnosis, and treatment.
Our clinical expertise permits us to help clients with:
- Long-term conditions like chronic obstructive pulmonary disease, diabetes, neurological, and others
- All types of learning and physical disabilities and mental health conditions
- Management of seizures
- Complex medication management
- Wound care
- Management of catheters/continence
- Oxygen therapy, optiflow therapy, nebulisers, and non-invasive ventilation

LIVE-IN CARE
When it’s needed, we can provide live-in care assistance. Live-in care allows you to receive person-centred, one-to-one support on your terms.
As the name suggests, one of our carers will stay with you in your home and provide care when you need it.
Our Registered Nurses are on call 24 hours a day too, giving you and your family peace of mind that help is always nearby.
CARE FOR MENTAL HEALTH CONDITIONS
Care Hub have an established mental health team that provides the highest standard of care and support to our clients.
They are trained to the highest standards and hold nationally recognised qualifications.
With 24-hour registered nurse and senior team support, our staff are fully equipped to support the range of needs that come with mental health conditions.


CARE FOR PEOPLE WITH LEARNING DISABILITIES
Care Hub works closely with Learning Difficulty services across Jersey and our dedicated team works with residential homes and charities to build and maintain their care skills and knowledge.
We have some fantastic case studies transitioning people into their own homes, creating independent living, improving quality of life, and helping our clients find work.
Autism Spectrum disorder care
Care Hub has a dedicated team specialising in supporting individuals with autism, ensuring our clients’ highest standard of care and assistance.
Our team members undergo extensive training and possess nationally recognized qualifications and specialist training in working with individuals on the autism spectrum, such as SPELL and Living with Autism training.
With round-the-clock support from registered nurses and a senior team, our staff is well-prepared to meet the diverse range of needs associated with autism.

Our approach to domiciliary care allows you to be at home, no matter what your needs are or how they change, with us by your side.
Every client is unique so we make sure you receive the right care from the right people at the right time.


PRIVATE NURSING
All Care Hub clients can access advice and signposting via our 24-hour on-call nursing service.
Benefits of Private Nursing
- Reduce hospital admissions when possible – have a professional examination and avoid calling out ambulance or GP services.
- Emergency treatment and examination for falls including wound care and first aid.
- This service can support clients at end of life, we can administer medications for pain and discomfort, spend time with the family and ensure all personal care needs are met too.
Other Private Nursing services
We can also provide additional services that include catheter changes, PEG tube changes, basic wound care, and clinical examinations. Clinical examinations include pulse, blood pressure, blood glucose levels, respirations and chest examination, weight, urinalysis, an overall health check, and a letter to your GP with our findings.

Once you’ve contacted us, we’ll come and meet you. We’ll talk about the sort of care you need; explain the range of services we provide and build a picture of how we can help you.
2. ASSESSMENT
We design a detailed care package that focuses on improving your health and wellbeing. We’ll discuss the details with you to make sure you’re happy and understand everything that’s in it.
3. INTRODUCTIONS
The Care Hub team members we choose to support you are matched to your needs. We will introduce them to you and make sure they fully understand the specifics of your care package and meet your needs.
4. REGULAR CHECK INS
Your relationship with Care Hub is important to us so we check in with you regularly to make sure everything is going well and make any changes that might be needed.
